Refusal of Artificial Life Support: If at any time I am diagnosed by qualified medical professionals as being in a terminal condition, permanently unconscious, or in a state where there is no reasonable expectation of recovery or meaningful quality of life, I direct that no artificial measures be used to prolong my life. This includes, but is not limited to, mechanical ventilation, resuscitation, artificial nutrition and hydration, or other invasive life-sustaining treatments. I request that my care focus on comfort, dignity, and pain relief only. Let me go. Let me be free.
